Chief Complaint

“I tried to speak but could not get the words out.”

History of Present Illness

73-year-old woman experienced sudden weakness in her right hand and difficulty speaking while at home with her husband. She has had several episodes of difficulty speaking and tingling in her right hand within the past two weeks that resolved within 15 minutes. The paramedics were called, and she was taken to the nearest stroke unit via ambulance.

CASE SUMMARY

Global Perspective

Stroke is the second leading cause of disability and death globally with the highest burden of disease seen in low- and middle-income countries. The global incidence of ischemic stroke in 2017 was 101.3 (91 – 113.6) per 100,000 population. The utilization of rtPA is approximately 10 – 15% in high-income countries compared to less than 2% in low- and middle-income countries. Treatment of ischemic stroke with rtPA and/or mechanical thrombectomy have been shown to be both effective at preventing disability and death and cost-effective. Global efforts to improve access to stroke treatments are ongoing and critically important to reduce the burden of disease of stroke.
